Job Description
Overview We are seeking a compassionate and dedicated Certified Home Health Aide to provide exceptional in-home care to clients with diverse needs. This role offers an opportunity to make a meaningful difference in patients' lives by delivering personalized support and assistance in a variety of settings, including assisted living, nursing homes, and community environments. The ideal candidate will possess strong patient observation skills, and familiarity with EMR/EHR charting systems. Certification as a Home Health Aide is required, along with a commitment to maintaining patient privacy and safety. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and document vital signs and patient conditions accurately using EMR/EHR systems Maintain detailed medical documentation in compliance with HIPAA regulations Observe and report changes in patient health or behavior promptly to healthcare professionals Work collaboratively with healthcare teams to deliver comprehensive home & community care Requirements Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) Experience working in home health, long-term care, nursing homes, or assisted living facilities preferred Knowledge of medical terminology and medical records management CPR certification required; additional certifications such as HIPAA training are advantageous Ability to work independently while following care plans and protocols Strong communication skills and compassionate bedside manner Ability to work flexible hours Must pass background checks and adhere to all safety protocols including infection control standards Join our team to provide compassionate, high-quality care that enhances the well-being of our clients while advancing your career in home health services.
