Full Job Description Tentative Shift:

8:00AM-12:00PM (Mondays, Wednesdays Fridays) Overview We are seeking a compassionate and dedicated Home Care Aide to provide essential support and care to individuals in their homes. This role is vital in enhancing the quality of life for clients, particularly those with developmental disabilities, long-term care needs, or those requiring hospice care. The ideal candidate will have a passion for helping others and the ability to create a nurturing environment that promotes independence and dignity. Duties Assist clients with daily living activities such as bathing, dressing, grooming, and exercise assistance. Implement and follow care plans tailored to individual client needs. Provide companionship and emotional support to enhance clients' well-being. Help clients with mobility, including lifting and transferring as needed. Monitor and document clients' health status and report any changes to the supervising nurse or healthcare professional. Support clients with medication reminders and adherence to prescribed treatments. Maintain a clean and safe environment for clients by performing light housekeeping tasks. Collaborate with family members, social workers, and healthcare professionals to ensure comprehensive care. Skills Experience working with individuals with developmental disabilities is highly valued. Strong background in home care or nursing home settings preferred. Knowledge of care plans and the ability to implement them effectively. Familiarity with long-term care practices and hospice care principles is beneficial. Excellent interpersonal skills with a focus on empathy and compassion. Ability to lift clients safely while ensuring their comfort and safety. Strong communication skills for effective interaction with clients, families, and healthcare teams. A commitment to providing high-quality care that respects the dignity of each individual.
