Job Description

Please note starting rates are $35/visit. Indeed does not allow us to post per visit numbers and only allows us to post them per hour. We are looking for a per diem Home Health Aide who loves caring for patients, has strong teamwork, and shares our vision in providing quality care to our community. Grace Community Home Health is a faith-based home health agency located in Mountain View, providing physician-ordered home health services to the surrounding Bay Area. The person in this position must be able to perform the following essential job functions with or without reasonable accommodations. Follow the instructions of the professional nurse/therapist in providing care.
  • Provide assistance with hygiene such as bathing, oral care, and dressing.
  • Perform and record accurate measurements (i.e. vital signs, or intake/output as instructed in the care plans).
  • Observe and report any safety hazards found in the client's home or any significant observations regarding the client.
  • Attend staff meetings, attend/complete 12 hours of annual in-services, and participates in orientation of new employees.
  • Report patient complaints to the RN.
  • Maintain patient confidentiality/adheres to HIPAA requirements and agency policy and procedures manual.
  • Possess the ability to follow written and oral instructions.
  • Report abnormal findings in patient's conditions as observed and per care plan to RN Perform the following task when delegated by the RN Non-invasive and non-sterile treatments Collection and documentation of vital signs, height, weight, I/O, environmental situations.
