The ideal candidate will possess a strong background in patient care, with experience working with individuals with developmental disabilities, geriatrics, and those requiring specialized dementia or Alzheimer's care. As a Certified Home Health Aide, you will play a vital role in supporting patients' daily living activities, ensuring their safety, comfort, and dignity while adhering to home healthcare regulations and maintaining strict confidentiality. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document patient conditions by observing vital signs, medication adherence, and overall health status using Electronic Health Record (EHR) systems or Medical Records documentation. maintaining compliance with HIPAA and home health care regulations. Support clients with meal preparation, light housekeeping tasks such as cleaning and laundry, and ensuring a safe living environment. Communicate effectively with clients, family members, and healthcare teams to ensure coordinated care plans are followed diligently. Maintain patient confidentiality at all times and adhere strictly to safety protocols such as first aid and basic life support (CPR). Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly desirable. Demonstrated proficiency in medical terminology and medical documentation practices. Experience working with individuals with disabilities or developmental challenges is preferred. Knowledge of home health agency operations and familiarity with home & community care standards. Proven ability to handle heavy lifting safely and operate patient transfer equipment such as Hoyer lifts or similar devices. Background in geriatrics or hospice care providing compassionate end-of-life support is advantageous. Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA), along with current CPR and first aid certification.
