Provide counseling, emotional support, crisis intervention, and care planning. Assist patients and caregivers with community resources, financial assistance programs, advance care planning, and long-term care resources. Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team to coordinate patient care. Educate patients and families regarding available support services and community resources. Advocate for patients to ensure appropriate access to healthcare and social services. Complete accurate, timely, and compliant documentation in accordance with Medicare regulations and company policies. Qualifications Master's Degree in Social Work (MSW) from an accredited program. Current California MSW license (LMSW/LCSW, as applicable). Home Health experience preferred, but not required. Strong communication, organizational, and interpersonal skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Valid California driver's license, reliable transportation, and current auto insurance.
