Job Summary Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals requiring assistance with daily activities and medical needs. In this vital role, you will provide personalized in-home support, ensuring safety, comfort, and dignity for patients across a variety of care settings. Your energetic approach and commitment to excellence will help promote independence and enhance quality of life for those you serve. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor and observe patient conditions closely, documenting vital signs and changes in health status accurately using Electronic Health Record (EHR) systems Administer medications and perform catheter care following medical instructions while maintaining strict adherence to HIPAA privacy regulations Support patients with memory care needs, including Alzheimer's and dementia care, creating a safe environment tailored to individual requirements Provide companionship and emotional support, working effectively with individuals with developmental disabilities or those in hospice or long-term care settings Prepare nutritious meals and assist with light housekeeping tasks including cleaning, laundry, and maintaining a safe home environment Document all patient interactions thoroughly in medical records, ensuring compliance with healthcare standards and protocols Qualifications Proven experience in home health care, assisted living, nursing homes, or long-term care environments Strong knowledge of medical terminology, vital signs monitoring, medication administration, and infection control practices Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as EHR systems training or hospice care are a plus Experience working with individuals with disabilities or special needs preferred Ability to operate medical equipment such as Hoyer lifts and perform tasks like catheter care confidently Excellent patient observation skills combined with compassionate caregiving abilities Ability to work independently within a home setting while maintaining professional standards Home Health aide/ LVN -
