Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report changes promptly to healthcare professionals Administer medications accurately following prescribed schedules and medical guidelines Support patients with medical procedures such as catheter care and wound management while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards Document patient care activities meticulously using Electronic Health Record (EHR) systems or medical records software; ensure accuracy and completeness of all documentation Facilitate safe patient transfers utilizing proper patient transfer methods and equipment like Hoyer lifts or patient transfer belts Provide companionship and emotional support, especially for clients with dementia or experiencing end-of-life stages; assist with meal preparation and light housekeeping tasks such as cleaning and laundry to maintain a safe living environment Follow all safety protocols by maintaining first aid readiness, performing CPR when necessary, and adhering to home health agency policies for client safety and confidentiality Skills Proven experience working with geriatric patients, individuals with developmental disabilities or dementia care needs Strong knowledge of medical terminology, medication administration procedures, and home health care regulations Skilled in patient observation techniques to identify changes in condition or behavior promptly Familiarity with EMR/EHR systems for accurate documentation of patient information and medical records Ability to assist with ADLs efficiently while maintaining patient dignity and comfort Experience working in assisted living facilities, nursing homes, or hospice environments is highly desirable Excellent communication skills for effective client interaction and collaboration with healthcare teams Capable of heavy lifting and physical tasks such as transferring patients using Hoyer lifts or other mobility aids Knowledge of basic life support (BLS), first aid procedures, CPR certification preferred Ability to work independently while following detailed care plans; dependable with a flexible schedule to meet client needs Join us in delivering exceptional home health care that enriches lives through compassionate service.
