Overview We are seeking compassionate and dedicated Home Health Aides to provide high-quality in-home care to clients across various settings. This role involves assisting individuals with daily activities, ensuring their safety, and supporting their overall well-being. The ideal candidate will have experience in patient care, familiarity with medical documentation, and a strong commitment to improving the quality of life for those they serve. Prior experience in assisted living, nursing homes, or working with individuals with developmental disabilities or dementia is highly valued. Responsibilities Provide personal care including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support. Monitor and observe patient conditions, including vital signs and behavioral changes, documenting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Assist with medication administration following prescribed protocols and maintaining compliance with HIPAA regulations to ensure patient confidentiality. Support clients with meal preparation, feeding, and ensuring proper nutrition according to dietary needs. Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment for clients. Assist with mobility using equipment like Hoyer lifts when necessary and ensure safe transfer techniques are followed. Provide companionship and emotional support to clients, especially those with dementia or memory care needs such as Alzheimer's disease. Collaborate with healthcare professionals to coordinate care plans and update medical records accurately. Support hospice care patients by providing comfort measures and respecting their dignity during end-of-life care. Requirements Proven experience in home health care, assisted living, nursing homes, or long-term care environments. Knowledge of medical terminology, medication administration, catheter care, hospice care, dementia care, Alzheimer's care, and memory care is preferred. Certification in CPR is required; additional certifications such as HIPAA training or specialized caregiving courses are a plus. Ability to work independently while following detailed care plans and protocols. Strong communication skills for effective interaction with patients, families, and healthcare teams. Experience working with people with disabilities or developmental challenges is advantageous. Familiarity with EMR/EHR systems for documentation purposes. Valid driver's license may be required depending on the position location and client needs.
