Compensation:
Hourly and paid-per-visit (PPV) hybrid model (negotiable based on experience, skill level, availability, etc.) Opportunity to accumulate overtime Job Purpose:
The Medical Social Worker is responsible for providing care and services to resolve social or emotional problems that are or are expected to be an impediment to the effective treatment of the patient's medical condition or rate of recovery. Qualifications:
A master's degree from a school of social work accredited by the Council on Social Work Education One year of social work experience in a healthcare setting Licensed as a Medical Social Worker by the state in which practicing, if applicable Training:
Experience in home health care preferred Responsibilities:
Assesses the social and emotional factors related to the patient's illness, need for care, response to treatment, and adjustment to care Assesses the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources Obtains available community resources to assist in resolving the patient's problems. Medicare does not cover the services of a medical social worker to complete or assist in Medicaid applications Provides education to patients and families regarding community resources Provides counseling services if required by the patient and included in the plan of care Furnishes medical social services to the patient's family members or caregiver on a short-term basis when necessary to support the patient's care plan Prepares clinical and progress notes Participates in developing the patient's plan of care Maintains accurate and timely documentation of care/services provided Notifies the Clinical Manager or Registered Nurse of changes in the patient's plan of care or clinical status Communicates with other team members regarding the patient's plan of care and updates them on changes Attends and participates in staff meetings, case conferences, in-services, and committee meetings as appropriate Reports incidents and grievances/complaints (patients and/or personnel) to the Clinical Manager Provides education and information to the patient and/or family members as appropriate Evaluates the patient's progress and effectiveness in meeting goals and outcomes Participates in the patient's discharge planning Participates in the agency's Quality Assessment and Performance Improvement (QAPI) program Provides staff education as requested by the Clinical Manager Adheres to agency policies and procedures Maintains confidentiality of all aspects of job performance Practices proper infection control and utilizes PPE as indicated Complies with all applicable federal, state, and local laws and regulations Performs other related duties as assigned Physical Demands:
