Job Description
We are looking for a Certified Home Health Aide (CHHA) to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus. Responsibilities Assist patients with personal care needs (bathing, mouth care, shampoo and hair care, transferring, dressing and undressing and ambulation) and provide housekeeping and therapy services to our patients in their home. Measure and record patients temperature, pulse, respiration, and blood pressure when ordered and/or directed by the RN, therapist, Clinical Manager or personal care plan. Work with other highly skilled clinicians via a multi- or interdisciplinary team approach. Other duties as directed by the RN, therapist, Clinical Manager, or personal care plan. Qualifications 1 year of recent CHHA experience with a home health agency preferred. Must have Current California Department of Public Health Home Health Aide Certification required. Current CPR/BLS certification.
