Monday-Thursday, 10:00 AM-3:00 PM Fridays Off About the Position Extended Care Services LLC is seeking a dependable, compassionate, and organized Direct Support Professional (DSP) to provide person-centered support to individuals participating in our Supported Community Connections (SCC) and Pre-Vocational programs. This is a consistent 20-hour-per-week, part-time position that combines community-based direct support, client transportation, Pre-Vocational services, required documentation in Therap, and light administrative responsibilities during designated office time. The ideal candidate enjoys helping individuals build independence, develop skills, participate in their community, and work toward their individual goals. This position is a great fit for someone who enjoys being active and working directly with people but is also comfortable completing documentation and administrative tasks.
Schedule Monday-Thursday:
10:00 AM-3:00 PM 20 hours per week Fridays off Candidates must be consistently available during these scheduled hours because the position includes scheduled client services and transportation responsibilities. Primary ResponsibilitiesSupported Community Connections (SCC) Transport individuals to and from scheduled community-based services and activities Provide person-centered support during community activities Encourage independence, choice, social engagement, and community inclusion Support individuals in developing and practicing community navigation and daily living skills Maintain awareness of individual health and safety needs while providing services Follow individual service plans and identified goals Complete accurate and timely service documentation Pre-Vocational Services Provide scheduled Pre-Vocational services at the Extended Care Services office Support individuals in developing employment-related and transferable workplace skills Provide appropriate coaching, prompting, and encouragement Promote independence and active participation Follow individual service plans and goals Document services and individual progress accurately Documentation & Administrative Support During designated office time, responsibilities may include: Entering service notes and required documentation into Therap Ensuring documentation is complete, accurate, and submitted on time Maintaining organized and confidential client records Completing assigned program-related administrative tasks Assisting with other light administrative responsibilities as assigned Qualifications Required High school diploma or equivalent Valid driver's license Reliable personal vehicle and current automobile insurance Comfortable transporting individuals receiving services Dependable and punctual Strong communication and interpersonal skills Basic computer and technology skills Ability to complete accurate electronic documentation Ability to work independently and as part of a team Compassionate, patient, and committed to person-centered services Ability to adapt when schedules or program needs change Preferred Previous experience supporting individuals with intellectual and developmental disabilities (IDD) Experience in human services, healthcare, caregiving, education, behavioral health, or community-based programs Experience providing community-based or Pre-Vocational services Experience using Therap or another electronic documentation system Previous IDD or Therap experience is preferred but not required for an otherwise qualified candidate who is willing and able to learn. Transportation & Mileage Transportation is an essential responsibility of this position. The employee will use their personal vehicle to transport individuals receiving services to approved community activities and program locations. Extended Care Services provides mileage reimbursement for approved business travel in accordance with agency policy. Ordinary commuting to and from work is not reimbursable.
