Role:
The Home Health Aide plays a critical role in providing compassionate and personalized care to individuals who require assistance with daily living activities in their own homes. This position ensures the safety, comfort, and well-being of patients by delivering high-quality support tailored to their unique health needs. The aide collaborates closely with healthcare professionals and family members to implement care plans effectively and monitor patient progress. By fostering a respectful and empathetic environment, the Home Health Aide helps enhance the quality of life for patients while promoting their independence. Ultimately, this role contributes significantly to the continuum of care within the health care services industry by supporting long-term and home care needs. As a Home Health Aide , you will: Provide direct patient care to patients under direction of the RN and according to the Aide Plan of Care and to include personal hygiene, ambulation, exercise, safe transfers and other supportive tasks as assigned. Appropriately report changes in the patient's condition. Complete appropriate documentation in a timely manner. Practice accepted infection control principles. Promote positive, supportive, respectful communication to patient/family and other employees. Provide a clean, safe and comfortable environment which promotes respect for patient, privacy and property. Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake. Promote the agency philosophy and administrative policies to ensure quality of care.
