The Role We are seeking a compassionate and dedicated Medical Social Worker (MSW) to provide home-based social work services to patients in the Longmont, Boulder, or Denver Metro area (30-mile radius). In this role, you will help patients and their families navigate healthcare challenges, connect them with community resources, and provide emotional support to improve their overall well-being. What You'll Be Doing Conducting assessments of patients' psychosocial status and developing individualized care plans Providing counseling and emotional support to patients and families facing chronic illness or end-of-life care Assisting with financial and resource planning , including insurance navigation and accessing community services Collaborating with the interdisciplinary team to ensure holistic, patient-centered care Advocating for patients' rights and coordinating services to enhance quality of life Educating patients and families about available resources, coping strategies, and care options What's In It for You? Requirements:

What Makes You a Great Fit Master's degree in Social Work (MSW) from an accredited program Valid Colorado Social Worker license Experience in home health, hospice, or medical social work preferred Strong knowledge of community resources, financial assistance programs, and healthcare systems Exceptional communication, problem-solving, and advocacy skills Licensed driver with current auto insurance and a good driving record
