Job Summary:

Join our dedicated team as a Direct Support Professional and help empower individuals with disabilities to live fulfilling, independent lives. In this full-time role, you will provide compassionate, person-centered support to participants in the Explorers Adult Day Program. Your efforts will promote community inclusion, personal growth, and safety while fostering meaningful connections. Responsibilities:

Support individuals in daily activities, including transportation to and from scheduled events and community outings, ensuring safety at all times. Assist in implementing personalized service and behavioral plans, helping participants achieve their personal goals with consistency and care. Encourage engagement in recreational, social, and community activities aligned with each individual's interests and plans. Promote social development by fostering the formation of meaningful relationships and networks. Support community inclusion by encouraging participation in local events and outings to enhance personal growth. Maintain punctuality and reliability across scheduled shifts, communicating schedule changes promptly. Protect individuals' rights by advocating for their dignity and ensuring respectful interactions. Support medication routines by reminding individuals to take prescribed medications; administer medications if certified. Observe and document any health or behavioral changes, reporting concerns promptly. Assist with personal care needs such as toileting, handwashing, and eating while promoting independence and dignity. Accurately document program data including service plans, progress notes, community logs, and assessments. Maintain detailed shift communication logs to ensure clear documentation of daily activities.

Qualifications:

Experience supporting individuals with developmental disabilities or related caregiving fields preferred; relevant experience will be considered. Valid driver's license with reliable transportation required. Ability to pass background checks and motor vehicle record screening. Strong organizational skills with attention to detail and multitasking ability. Excellent interpersonal skills for effective communication with colleagues and participants. Emotionally mature, discreet, and committed to confidentiality. Proficient with technology such as computers and email platforms. Effective written and verbal communication skills. CPR/First Aid certification preferred or obtainable upon hire. QMAP certification preferred or obtainable upon hire. We are committed to fostering an inclusive environment that values diversity.
