Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs, recognizing changes, and reporting findings accurately using electronic medical record (EMR) systems Support with medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards Provide specialized care such as Alzheimer's and dementia care, memory care, end-of-life support, and hospice assistance Operate patient transfer methods safely using equipment like Hoyer lifts or other transfer devices; assist with patient transfers and mobility exercises Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients Document medical records accurately and maintain detailed medical documentation for each client's care plan Qualifications Prior experience working with individuals with developmental disabilities, geriatrics, or in assisted living settings is highly preferred Knowledge of medical terminology, catheter care, CPR, first aid, and basic life support (BLS) certifications required or obtainable upon hire Experience with EMR systems and medical documentation practices essential for accurate record keeping Ability to lift heavy objects safely and perform physically demanding tasks as needed for patient transfer and mobility assistance Strong communication skills for effective client interaction and collaboration with healthcare teams Familiarity with home health care regulations, HIPAA compliance, and patient confidentiality standards necessary for quality care delivery Previous experience in home health agency settings or long-term care environments is advantageous Join us today to be part of a passionate team committed to delivering exceptional personal care that truly makes a difference!
