The ideal candidate will have experience working with hospice, home health, dementia care, and developmental disabilities, and more. The Social Worker will be responsible for providing care and support to patients and their families, as well as conducting research and developing systems to improve patient outcomes. Qualifications Required Bachelor's or Master's degree in Social Work or related field Experience working with adults and elderly populations Driver's license and car insurance Strong communication skills Ability to work independently as well as part of a team LCSW or LMSW License What You'll Do Assesses the psychosocial status of patients and families/caregivers related to the patient's medical condition and environment and communicates findings to the registered nurse and other members of the interdisciplinary group Conducts assessments of patients' needs and develop care plans Provides individual and group counseling to patients and their families Collaborates with healthcare professionals to ensure the best possible care for patients Provides information and referral services for patients and families/caregivers regarding practical and environmental needs Provides information to patients and families/caregivers and community agencies Serves as a liaison between patients and families/caregivers and community agencies Develops systems to improve patient outcomes Conducts research to identify best practices in social work Provides education and training to staff on social work topics Maintains accurate records of patient care Compensation Hourly Rate $70 - $75 an hour Apply Today!
