Job Description
TriSource is seeking a dedicated and compassionate Direct Support Professional for a Day Program for adults in Norwich, CT! You will provide personalized support and assistance to individuals with developmental disabilities, ensuring they have meaningful, engaging, and safe experiences throughout their day. Your efforts will foster independence, promote well-being, and enhance the quality of life for those we serve. The ideal candidate will demonstrate patience, professionalism, and a strong commitment to person-centered care. Monday-Friday 6:45am-2:00PM Responsibilities Assist individuals with activities of daily living (ADLs), including grooming, dressing, toileting, and mobility support. Implement and follow individualized care plans tailored to each person's needs and goals. Monitor clients' health by observing vital signs, recognizing changes in condition, and reporting concerns promptly. Support medication administration in accordance with HIPAA regulations and care protocols. Facilitate engaging activities that promote socialization, skill development, and community integration. Provide transportation assistance to appointments, community outings, or recreational activities. Maintain a clean and safe environment by performing routine cleaning tasks and adhering to infection control standards. Document observations, care provided, and any incidents accurately in compliance with privacy regulations. Collaborate with team members, families, and healthcare professionals to ensure comprehensive support. Assist with meal preparation and feeding as needed while promoting healthy eating habits. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings preferred. Certification in CPR and first aid is required Clean background PMT Preferred but not required Valid drivers license
