  • DSPs) to empower individuals with intellectual and developmental disabilities to live, learn, work, and play with independence and purpose.
For over 40 years, Horizons has fostered dignity, connection, and inclusion. As a Support Coach, you'll build real relationships and support people in achieving their goals and living life on their own terms.
What You'll Do:
Support individuals with daily living tasks—such as meals, hygiene, and household routines—with respect and encouragement. Help maintain a welcoming home environment and assist with meal prep in accordance with any dietary needs. Assist individuals in working toward personal goals identified in their Individual Service Plan (ISP). Get to know each person's unique interests, strengths, and communication styles. Administer medications according to physician's orders and DDS guidelines. Respond with compassion and training in moments of challenge, ensuring emotional and physical safety. Communicate clearly, respectfully, and in ways that center the individual's voice. Document your work thoughtfully and participate in staff development and training. Keep your manager informed of anything notable related to the individual's well-being.
