Job Description
Job Overview We are seeking compassionate and dedicated Caregivers/Home Health Aides to provide exceptional support and personalized care to individuals in their homes or community settings. In this vital role, you will assist clients with daily activities, monitor their health status, and promote independence and well-being. Your energetic approach and attention to detail will make a meaningful difference in the lives of those you serve. This position offers a rewarding opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those requiring specialized care such as dementia or Alzheimer's support. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient health by taking vital signs, observing symptoms, and reporting changes to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with medication administration following care plans. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces. Use specialized equipment like Hoyer lifts or wheelchairs to ensure safe transfers and mobility assistance. Support clients with memory care needs, including dementia or Alzheimer's care, ensuring a secure environment. Document all care activities accurately while adhering to HIPAA regulations to protect patient confidentiality. Collaborate with healthcare teams to develop and implement personalized care plans that meet each client's unique needs. Provide emotional support and companionship to foster a positive and engaging environment for clients. Follow safety protocols during patient monitoring and respond promptly to emergencies or urgent situations. Experience Prior experience working in home health, assisted living facilities, nursing homes, or group homes is highly preferred. Demonstrated knowledge of patient observation techniques, vital signs measurement, and medication administration. Experience supporting individuals with developmental disabilities, autism spectrum disorder, or dementia is advantageous. Familiarity with HIPAA compliance and maintaining confidentiality in healthcare settings is essential. Hands-on experience with caregiving tasks such as cooking, cleaning, and assisting with ADLs is required. Certification in CPR and First Aid is strongly preferred; ongoing training in hospice or memory care is a plus.
