Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote dignity and independence. Monitor patients' health by observing vital signs, reporting changes, and maintaining accurate care plans in accordance with HIPAA regulations. Support medication administration and manage patient transfer methods using equipment such as Hoyer lifts and manual handling techniques. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to ensure proper nutrition. Provide companionship and emotional support while engaging clients in social activities or recreational pursuits to promote mental well-being. Maintain a clean and safe environment through regular cleaning duties and adherence to infection control protocols. Document patient observations, care activities, and communication with healthcare professionals to ensure continuity of care. Skills Proven experience working with individuals with developmental disabilities, neurodevelopmental disorders, or dementia including Alzheimer's care and memory care. Knowledge of patient monitoring techniques, medication administration, and emergency response procedures such as CPR and first aid. Familiarity with care plans, resident's rights, HIPAA compliance, and documentation standards within home health or long-term care settings. Ability to perform heavy lifting safely using proper manual handling techniques; experience operating assistive devices like Hoyer lifts is preferred. Strong communication skills for client interaction, family engagement, and collaboration with healthcare teams. Experience working in group homes, assisted living facilities, nursing homes or hospice environments is highly desirable. Capable of assisting with complex needs including hospice care, geriatrics, social work responsibilities, and supporting individuals with autism or intellectual disabilities.
