Job Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and proactive support will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance to individuals in their homes, promoting independence, safety, and well-being. Your energetic approach and attention to detail will help create a comfortable, respectful environment for those you serve. This position offers an opportunity to work closely with diverse patient populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. Your efforts will support their health goals while ensuring compliance with all home healthcare regulations and privacy standards. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly Administer medications and treatments following prescribed instructions while maintaining accurate medical documentation Support patients with specialized care needs such as catheter care, Alzheimer's and dementia care, and end-of-life assistance Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions and updates securely Use proper patient transfer methods and equipment like Hoyer lifts to ensure safe mobility assistance Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and meal preparation in accordance with health standards Communicate effectively with clients and their families to provide reassurance and education on care plans while respecting HIPAA guidelines for confidentiality Follow home healthcare regulations diligently to ensure compliance during all aspects of patient care Qualifications Proven experience working as a Home Health Aide or in assisted living, nursing homes, or hospice settings Strong knowledge of medical terminology, patient care procedures, and safety protocols including first aid, CPR, and basic life support (BLS) certifications Experience working with individuals with developmental disabilities or geriatrics is highly preferred Ability to observe patient conditions carefully and report findings accurately using EMR/EHR systems Familiarity with home health agency operations and home & community care standards Physical stamina for heavy lifting, patient transfers using methods like Hoyer lifts or transfer belts, and long periods of standing or walking Excellent communication skills for engaging with patients, families, and healthcare teams effectively Commitment to maintaining patient confidentiality in accordance with HIPAA regulations Valid driver's license may be required depending on the service area; experience with live-in employment is a plus Embark on a rewarding career where your dedication transforms lives.
