Responsibilities Deliver personal care assistance, including helping clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patients' health by observing vital signs and overall condition, promptly reporting any concerns to healthcare professionals Assist with medication administration following care plans and HIPAA regulations to ensure confidentiality and safety Support clients with meal preparation and nutritional needs, ensuring dietary restrictions are followed accurately Utilize proper patient transfer methods and manual handling techniques, including the use of Hoyer lifts and patient transfer devices Maintain detailed documentation of care provided, patient observations, and adherence to individualized care plans Provide transportation assistance for appointments or community outings while ensuring safety at all times Support clients with cognitive impairments such as dementia or Alzheimer's disease through specialized memory care techniques Assist with housekeeping tasks like cleaning and maintaining a safe living environment in accordance with health standards Engage in patient monitoring during home health visits or hospice care to ensure ongoing comfort and safety Experience Candidates should possess prior experience working in home health, assisted living facilities, nursing homes, or group homes. Experience working with individuals with disabilities—including developmental disabilities, neurodevelopmental disorders, or intellectual disabilities—is highly valued. Familiarity with dementia care, Alzheimer's care, hospice support, and managing patients with complex medical needs is essential. Applicants should demonstrate proficiency in patient observation, medication administration, vital signs measurement, and emergency response procedures such as CPR and first aid. Prior experience using patient transfer equipment like Hoyer lifts or patient personal transportation aids is preferred. A background in geriatrics or long-term care environments will enhance your ability to provide exceptional support. Strong communication skills for client interaction and teamwork are vital for success in this role.
