In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and well-being. This position offers a rewarding opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or palliative care. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and dignity. Monitor patient health by observing vital signs like blood pressure, pulse, respiration, and temperature; report any abnormalities promptly. Support medication administration following prescribed instructions while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards. Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems; ensure all medical records are up-to-date and complete. Help with mobility and transfers using patient transfer methods such as Hoyer lifts or other assistive devices to prevent injury. Provide companionship and emotional support, especially for clients with dementia or Alzheimer's disease, fostering a safe and supportive environment. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for the patient. Assist with specialized care needs such as catheter care, wound management, or end-of-life support as required by the patient's condition. Follow all home health care protocols related to infection control, safety procedures, and emergency response including first aid and CPR training. Communicate effectively with clients' families and healthcare teams to coordinate care plans and update on patient progress. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred. Demonstrated experience caring for geriatric patients or individuals with developmental disabilities is essential. Familiarity with medical terminology, medication administration procedures, and medical documentation practices is required. Experience working with individuals experiencing memory care challenges such as dementia or Alzheimer's disease is advantageous. Knowledge of home health agency operations, hospice care practices, and home & community care standards is desirable. Ability to perform heavy lifting safely when necessary for patient transfers or mobility assistance. Previous exposure to EMR/EHR systems for documentation purposes is a plus.
