Job Description

Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse needs. In this vital role, you will assist individuals with daily living activities, monitor health status, and promote independence while ensuring safety and comfort. Your energy and attentiveness will make a meaningful difference in the lives of those you serve, supporting their well-being and quality of life in a respectful, professional manner. Responsibilities Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting to promote personal hygiene and comfort. Monitor and observe patients' health status, including vital signs like blood pressure, pulse, respiration, and temperature, documenting changes accurately. Support medication administration following prescribed schedules and protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards. Provide specialized care for clients with Alzheimer's disease, dementia, or other memory care needs, implementing personalized care plans. Assist with mobility and patient transfer methods using equipment such as Hoyer lifts or other safe transfer techniques to prevent injury. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Maintain detailed medical documentation within electronic health record (EHR) systems or medical records to ensure accurate tracking of patient progress and compliance. Skills Proven experience working with individuals with disabilities, developmental disabilities, or geriatric patients in home healthcare or assisted living settings. Strong knowledge of medical terminology, catheter care, medication administration, and basic life support (BLS) including CPR. Familiarity with home health care assistance tools such as patient transfer methods and Hoyer lifts; experience with hospice or end-of-life care is a plus. Excellent patient communication skills with the ability to provide compassionate personal care assistance while respecting patient confidentiality under HIPAA guidelines. Ability to work independently in a home health environment while adhering to safety protocols and home health agency standards. Capable of heavy lifting when necessary and performing physical tasks associated with caregiving duties safely. Experience working with electronic medical records (EMR/EHR systems) for documentation purposes; familiarity with home & community care regulations is preferred.
