Responsibilities Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, recording vital signs and changes in health status accurately in electronic health record (EHR) or electronic medical record (EMR) systems Administer medications and manage catheter care following prescribed instructions and safety protocols Support patients with memory care needs such as Alzheimer's disease and dementia, ensuring a safe environment and engaging in therapeutic activities Perform patient transfers using appropriate methods such as Hoyer lifts or transfer belts, ensuring safety for both patient and caregiver Provide end-of-life care with compassion, dignity, and respect for patient wishes, supporting families through difficult times Maintain patient confidentiality in compliance with HIPAA regulations while documenting medical records and care activities thoroughly Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment Experience Prior experience providing care in the home care setting is highly preferred. Experience in other healthcare setting can be of great benefit to in-home patients. Demonstrating knowledge and hands-on abilities will be necessary for a position in home health care. Proven ability to communicate effectively with patients, families, and healthcare teams while maintaining professionalism and empathy Hands-on experience with patient transfer methods such as Hoyer lifts or transfer boards is a plus.
