This vital role involves assisting clients with daily activities, monitoring health status, and ensuring their safety and comfort. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting. hours are from M-F 7am-4pm Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately in care plans while adhering to HIPAA regulations to ensure confidentiality. Provide medication administration under supervision or as trained, following prescribed schedules and safety protocols. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Support clients with personal transportation needs for appointments or community outings, ensuring safe transfer and mobility assistance. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are met. Document all care activities thoroughly, communicate effectively with clients and team members, and participate in ongoing training related to dementia care, Alzheimer's care, memory care, autism support, neurodevelopmental disorders, and other specialized areas of patient care. Qualifications Previous experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred. Knowledge of patient observation techniques, patient monitoring, and the ability to recognize signs of distress or deterioration. Experience with specialized care such as dementia care, Alzheimer's care, hospice care, or intellectual disabilities support is a plus. Certification in First Aid and CPR is required; additional certifications such as HIPAA training or medication administration certification are advantageous. Ability to perform heavy lifting safely using proper manual handling techniques; experience working with Hoyer lifts is beneficial. Strong communication skills to effectively interact with clients with developmental disabilities or neurodevelopmental disorders and their families. Demonstrated ability to follow detailed care plans while maintaining confidentiality according to HIPAA standards.
