Job Summary We are seeking a dedicated Home Health Aide to provide compassionate, high-quality care to clients in Fleming Island. This part-time position offers consistent morning hours from Monday through Thursday, 9:00 am to 1:00 pm. The ideal candidate will possess a strong background in home healthcare assistance, with experience working with geriatric patients, individuals with disabilities, and those requiring memory or Alzheimer's care. You will play a vital role in supporting clients with their daily needs while ensuring safety, comfort, and dignity. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Observe and monitor patient conditions, documenting changes accurately using Electronic Health Record (EHR) systems or medical documentation tools. Support medication administration and manage vital signs under supervision or as trained. Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation. Follow home health care regulations, maintaining patient confidentiality in compliance with HIPAA standards. Assist with light housekeeping tasks such as laundry, cleaning, and meal preparation to ensure a safe and comfortable environment. Use proper patient transfer methods and equipment like Hoyer lifts when necessary to prevent injury. Experience Candidates should have prior experience working in home healthcare settings or assisted living environments. Experience working with individuals with developmental disabilities, dementia or Alzheimer's disease, and hospice or end-of-life care is highly desirable. Familiarity with medical terminology, catheter care, and basic life support (CPR/First Aid) is essential. Applicants must demonstrate competence in patient transfer methods, medication management, and adhering to home health agency standards. Prior use of EMR/EHR systems for medical records documentation is preferred. A background involving geriatric care or long-term care environments will be considered an asset.
