Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse needs. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote independence and hygiene. Monitor patient health by observing vital signs, reporting changes, and maintaining detailed medical documentation using Electronic Health Record (EHR) systems. Support medication administration following home healthcare regulations and physician instructions, ensuring accuracy and safety. Provide specialized care for clients with Alzheimer's disease, dementia, or other memory impairments, including memory care techniques and safety measures. Perform patient transfers using appropriate methods such as Hoyer lifts or transfer belts, ensuring safety for both client and caregiver. Assist with light housekeeping duties including cleaning, laundry, meal preparation, and maintaining a safe living environment. Communicate effectively with clients, family members, and healthcare teams to coordinate care plans while respecting patient confidentiality under HIPAA guidelines. Skills Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or end-of-life care. Strong knowledge of medical terminology, catheter care, wound care, and vital signs monitoring. Familiarity with EMR/EHR systems for accurate documentation of patient information and medical records. Ability to perform patient personal care assistance with compassion and professionalism. Knowledge of home health care assistance regulations and best practices in home & community care environments. Competence in patient transfer methods including Hoyer lifts and other mobility aids; experience working with heavy lifting is preferred. Certifications such as First Aid and Basic Life Support (BLS), CPR are required; additional certifications in dementia or Alzheimer's care are a plus. Excellent communication skills to foster positive client relationships and collaborate effectively within healthcare teams.
