Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and record vital signs such as blood pressure, pulse, respiration, and temperature accurately using EMR (Electronic Medical Records) systems Provide medication reminders and assist with medication administration following prescribed instructions Support patients with medical procedures like catheter care and wound management while maintaining strict adherence to infection control protocols Observe and report changes in patient condition or behavior promptly to supervising healthcare professionals Assist with meal preparation, feeding, light housekeeping, laundry, and cleaning tasks to ensure a safe and comfortable living environment Use proper patient transfer methods including Hoyer lifts or other assistive devices to ensure safe mobility and prevent injury Skills Proven experience working with geriatric patients or individuals with disabilities in home healthcare or assisted living settings Strong knowledge of medical terminology, home health care regulations, HIPAA compliance, and patient confidentiality standards Skilled in patient care techniques such as vital signs measurement, CPR certification, first aid training, and basic life support (BLS) Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Ability to communicate effectively with clients, families, and healthcare teams to ensure comprehensive care plans are followed Experience working with individuals with dementia or Alzheimer's disease, including memory care strategies and end-of-life support Physical capability for heavy lifting, patient transfers using methods like Hoyer lifts or transfer belts, and performing tasks that require mobility assistance Join us in delivering exceptional home health care assistance that enriches lives through compassionate service.
