Responsibilities Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and recognizing changes in condition Support medication administration following prescribed instructions and maintaining accurate medical documentation Help with light housekeeping tasks including cleaning, laundry, and meal preparation to ensure a safe and comfortable environment Use proper patient transfer methods and equipment such as Hoyer lifts or transfer belts to ensure safety during mobility assistance Maintain patient confidentiality in accordance with HIPAA regulations and home healthcare standards Communicate effectively with patients, families, and healthcare teams to coordinate care plans and report concerns promptly Qualifications Prior experience working as a Home Health Aide or in assisted living, nursing homes, or hospice care settings preferred Knowledge of medical terminology, medical records management, and electronic health record (EHR) systems Demonstrated ability to perform patient observation, assist with ADLs, and provide compassionate personal care assistance Certification in First Aid and Basic Life Support (BLS), including CPR training required or willing to obtain upon hire Experience working with individuals with developmental disabilities, dementia (including Alzheimer's), or geriatric patients is highly valued Ability to lift heavy objects safely and perform physical tasks associated with patient transfer methods such as Hoyer lifts or similar equipment Familiarity with home health care regulations, medication administration protocols, and hospice or end-of-life care practices is advantageous Join us in delivering exceptional home health care that empowers clients to live healthier, happier lives.
