Responsibilities Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document patient vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring timely communication with healthcare providers. Administer medications following prescribed instructions while maintaining detailed medical documentation in electronic health record (EHR) systems or medical records. Support patients with specialized needs such as Alzheimer's care, dementia care, hospice care, or end-of-life assistance with compassion and professionalism. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment aligned with home health care standards. Observe and report changes in patient condition or behavior promptly while maintaining patient confidentiality in compliance with HIPAA regulations. Use proper patient transfer techniques and equipment like Hoyer lifts to ensure safe mobility and prevent injury for both patients and caregivers. Requirements Proven experience working with individuals with disabilities, geriatrics, or in long-term care settings such as nursing homes or assisted living facilities. Knowledge of medical terminology, medication administration protocols, catheter care, and vital signs monitoring. Certification in CPR (Cardiopulmonary Resuscitation) and basic first aid; additional certifications such as BLS (Basic Life Support) are preferred. Familiarity with EMR/EHR systems for accurate documentation; experience working within home healthcare regulations is highly desirable. Ability to perform heavy lifting when necessary and work independently or as part of a team delivering home & community care assistance. Excellent communication skills for effective client interaction and coordination with healthcare professionals. Valid driver's license and reliable transportation may be required depending on client location; live-in employment experience is a plus.
