RESPONSIBILITIES

Assess patients' psychosocial needs and social determinants that may affect their health and ability to remain safely at home. Provide psychosocial support to patients and families. Assist patients and caregivers in identifying and accessing appropriate community resources. Provide information regarding available social service programs and resources. Assist with care planning and coordination of appropriate services. Identify barriers to care and communicate concerns to the interdisciplinary team. Provide supportive counseling within the social worker's scope of practice. Assist patients and families with adjustment to illness, disability, changes in functional status, and other psychosocial concerns. Coordinate with community agencies and other appropriate resources. Communicate significant findings and changes with the appropriate members of the healthcare team. Complete accurate and timely clinical documentation. Participate in interdisciplinary communication and care coordination. Follow Medicare, Florida, agency, infection prevention, patient safety, and HIPAA requirements. Respect patient confidentiality, dignity, culture, preferences, and individual needs.

QUALIFICATIONS

Appropriate Florida Social Work license/credential for the position. Master's degree in Social Work ( MSW ) from an accredited program, as applicable to the position. Current BLS/CPR certification , if required by agency policy and applicable position requirements. Strong communication, assessment, organization, and interpersonal skills. Ability to work independently in the home health setting. Reliable transportation and ability to travel to patient homes throughout Broward County. Ability to complete required documentation accurately and on time. Home health experience is preferred but not required.
