In this vital role, you will assist clients with daily activities, monitor their health status, and ensure their comfort and safety. Your energetic approach and attention to detail will help promote independence, dignity, and quality of life for individuals receiving home healthcare services. This position offers an opportunity to make a meaningful difference in the lives of patients while working within a supportive and dynamic team. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and equipment like Hoyer lifts when necessary Observe and document patient conditions, including vital signs, medication adherence, and changes in health status using Electronic Health Record (EHR) systems while maintaining strict confidentiality in accordance with HIPAA regulations Provide medication reminders and assist with medication administration under the supervision of healthcare professionals Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, end-of-life care, and working with individuals with developmental disabilities or disabilities Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients Assist with medical procedures such as catheter care, taking vital signs, medical documentation, and basic life support (BLS) or CPR when required Facilitate effective communication with clients and family members to ensure personalized care plans are followed and needs are met Qualifications Proven experience as a Home Health Aide or in assisted living/nursing home environments with familiarity in geriatric and hospice care Bilingual fluency in Spanish is required; strong communication skills in English are also essential Knowledge of home healthcare regulations, medical terminology, patient transfer techniques, and HIPAA compliance Certifications in First Aid and Basic Life Support (BLS/CPR) are preferred or must be obtained upon hire Experience working with individuals with disabilities or special needs is highly desirable Ability to lift heavy objects safely and perform physical tasks associated with caregiving responsibilities Strong interpersonal skills to build trusting relationships with patients and their families Join our team to deliver compassionate home health services that truly make a difference!
