Job Description

Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse needs. In this vital role, you will support individuals with their daily activities, promote independence, and ensure their safety and comfort. This paid position offers an opportunity to work closely with clients, families, and healthcare teams to deliver personalized care that meets each individual's unique needs. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient conditions regularly, documenting changes accurately in electronic health record (EHR) or medical documentation systems Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations and patient confidentiality standards Perform vital signs assessments such as blood pressure, pulse, respiration, and temperature checks to inform ongoing care plans Provide specialized care for clients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Assist with transfers using patient transfer methods like Hoyer lifts or other equipment safely and efficiently Support end-of-life or hospice care by offering comfort measures and emotional support to patients and their families Requirements Proven experience working as a Home Health Aide or in assisted living, nursing home, or long-term care settings Strong knowledge of medical terminology, home healthcare regulations, and patient care best practices Demonstrated ability to perform patient personal care assistance with compassion and professionalism Familiarity with EMR (Electronic Medical Records) systems and medical documentation procedures Certification in CPR and basic life support (BLS), with first aid training preferred Ability to lift heavy objects safely and perform physical tasks such as cleaning, laundry, meal preparation, and light housekeeping as needed Experience working with individuals with disabilities or developmental challenges is highly valued Join us in delivering high-quality home health care that transforms lives!
