As a Home Health Aide Overnight, you will be the trusted caregiver supporting clients with their daily needs while maintaining a safe and nurturing environment. Responsibilities Monitor patients' health status by observing vital signs, mental state, and overall condition throughout the night. Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Ensure patient safety by conducting regular checks for falls or other emergencies and responding promptly to any concerns. Administer medications as prescribed, document medical information accurately using Electronic Health Record (EHR) systems or medical records. Support patients with end-of-life or hospice care needs with compassion and respect for their dignity. Operate patient transfer methods such as Hoyer lifts or other assistive devices safely to prevent injury. Maintain strict adherence to home healthcare regulations and HIPAA guidelines to protect patient confidentiality at all times. Requirements Proven experience working with individuals with disabilities, dementia care, Alzheimer's care, or geriatrics in home healthcare or nursing home settings. Knowledge of medical terminology, catheter care, medication administration, vital signs monitoring, and basic life support (BLS) including CPR. Familiarity with EMR/EHR systems for accurate documentation of patient care activities. Ability to perform heavy lifting and physical tasks required for patient transfers and mobility assistance safely. Strong communication skills to effectively interact with patients, families, and healthcare teams. Valid certification in first aid and CPR; additional certifications in hospice or dementia care are a plus. Understanding of home health care assistance standards and home & community care experience preferred. Ability to work overnight shifts reliably while maintaining high-quality patient care standards.
