POSITION SUMMARY

The Social Worker is responsible for assessing residents' psychosocial needs, coordinating services, facilitating care planning, and providing emotional support to residents and families throughout their stay. The Social Worker works collaboratively with the interdisciplinary team to promote resident well-being, support successful transitions of care, and ensure compliance with applicable regulatory requirements.

JOB REQUIREMENTS

Experience & Skills Minimum of 2 years of related experience required. At least 1 year of experience in a healthcare setting, such as a hospital, clinic, rehabilitation center, or similar environment. Skilled nursing experience is required. Excellent problem-solving skills. Ability to utilize a customer-focused approach to problem solving and goal setting. Excellent communication and listening skills. Enthusiastic, resourceful, and organized. Strong personal awareness of nonverbal communication. Computer proficiency, including Microsoft Office skills. Education Bachelor's Degree in Social Work, Psychology, Gerontology, or Sociology from an accredited institution is required.

RESPONSIBILITIES

Provide emotional support to residents and their families throughout their stay. Collaborate with the interdisciplinary team to promote resident well-being. Promote a customer-focused approach to problem solving and goal setting. Ensure compliance with applicable regulatory requirements. Maintain appropriate documentation and utilize computer systems, including Microsoft Office, as required.

The ideal candidate will be a Bachelor's-level Social Worker with at least 2 years of related experience, including a minimum of 1 year in a healthcare setting. Skilled nursing experience is required. The successful candidate will demonstrate strong communication, listening, problem-solving, and organizational skills, along with a customer-focused approach to working with residents, families, and interdisciplinary team members.
