Duties Assist patients with Activities of Daily Living (ADLs), including bathing, grooming, dressing, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document vital signs, medication administration, and patient observations accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with home healthcare regulations and HIPAA confidentiality standards. Support patients with memory care needs, Alzheimer's disease, dementia care, and end-of-life care by providing compassionate companionship and maintaining a safe environment. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a hygienic living space to promote overall health. Assist with medical procedures like catheter care, wound care, and basic first aid or CPR when necessary. Communicate effectively with clients and their families to address concerns and provide updates on patient status while maintaining professional patient care standards. Support patient transfer methods safely using appropriate techniques and equipment to prevent injury for both the patient and caregiver. Experience Previous experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred. Demonstrated knowledge of medical terminology, medication management, vital signs monitoring, and basic life support (BLS) or CPR certification. Experience working with individuals with disabilities, developmental disabilities, geriatrics, or those requiring hospice or memory care services. Familiarity with home health agency operations, home & community care environments, and home health documentation systems such as EMR/EHR platforms. Ability to handle heavy lifting safely when assisting patients with mobility or transfers; experience operating equipment like Hoyer lifts is a plus. Strong communication skills for effective client interaction and collaboration with healthcare teams while maintaining patient confidentiality per HIPAA guidelines.
