Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' health status by recording vital signs such as blood pressure, pulse, temperature, and respiration Administer medications accurately following medical instructions and maintain detailed medical documentation in Electronic Health Record (EHR) systems Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, and end-of-life assistance with compassion and patience Utilize proper patient transfer methods and equipment like Hoyer lifts to ensure safe movement and prevent injury Maintain patient confidentiality in accordance with HIPAA regulations while managing medical records and documentation Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment Assist with medical procedures such as catheter care and basic wound management under supervision or as trained Communicate effectively with clients' families and healthcare teams to coordinate care plans and report changes in condition Experience Prior assisted living or home healthcare experience preferred; experience working with individuals with disabilities or developmental disabilities is highly valued Knowledge of home health care regulations, medical terminology, and patient safety protocols such as First Aid and Basic Life Support (BLS) certifications required Hands-on experience with patient transfer methods including the use of Hoyer lifts or similar equipment is advantageous Demonstrated ability to work with geriatric patients in settings like nursing homes or hospice environments preferred Strong understanding of HIPAA compliance, patient confidentiality practices, and home & community care standards Experience working with patients requiring dementia or Alzheimer's care enhances your ability to deliver specialized support Ability to perform physically demanding tasks such as heavy lifting safely while maintaining a caring attitude Join us in providing exceptional home health care that empowers individuals to live comfortably and independently.
