Home Health Medical Social Worker (MSW) — PRN Preferred Care Home Health is seeking a compassionate, experienced PRN Medical Social Worker to support patients and families navigating health challenges in the comfort of their own homes. This is a flexible, per-diem opportunity ideal for an MSW who wants meaningful, patient-centered work without a full-time commitment. As part of our home health team, you'll help patients cope with the emotional, social, and practical realities of illness or disability — connecting them to resources, supporting care planning, and empowering them to live as independently and fully as possible. What You'll Do Support Patients & Families Assess the social and emotional factors affecting a patient's health and recovery Help patients understand, accept, and follow their physician's care recommendations Guide patients and families through personal and environmental challenges that impact healing Provide services aimed at restoring patients to their optimum level of well-being Coordinate Care & Connect Resources Contribute to the patient's Plan of Care as part of the interdisciplinary team Identify and connect patients with community agencies, family support, and other resources Support discharge planning and serve as a consultant to fellow health care staff Document & Collaborate Complete clinical and progress notes within required timeframes Participate in team conferences, in-services, and Performance Improvement initiatives Uphold strict confidentiality for all patient, employee, and Agency information
What You Bring Education:
Master's degree in Social Work from a CSWE-accredited program (required)
Experience:
Minimum of 1 year of social work experience in a health care setting (home health experience a plus)
License:
Florida social work license preferred, not required
Skills:
Strong assessment, communication, documentation, and care coordination abilities; comfortable managing multiple priorities and working independently
