Full Job Description Job Summary:
Offers medical services for person's who need a little extra assistance. Services can include any of the following; companionship, meal preparation, grooming, dressing, transportation bed/ tub transfers, housekeeping, transportation to appointments, laundry, shopping, running errands, recreational activities.
Essential Functions:
Follow the established Service Plan/Care Plan to provide overall attention to the medical needs of clients; assist with instrumental activities of daily living; provide companionship to the client, including acting as a conversationalist and good friend as well as grooming and hygiene, transfers, and assists with lifts, wheelchairs, walkers, and other assistive devices. Provide assistance with Medical Appointments/Transportation Bed/Tub Transfers (HHA). Provide assistance with Grooming and Hygiene (HHA) Family Respite Care. Assist with lifts, wheelchairs, walkers, and other assistive devices. Assist in and out of bathroom and other activities of daily living. Assist with Turning in bed (HHA) feeding. Grooming and Dressing. Provide light housekeeping duties such as laundry and linen service, to include washing, ironing, and folding clothes. Prepare and serve meals and snacks as needed. Provide shopping service to client(s) to include grocery and clothes shopping. Provide transportation for the client(s), which can include doctor appointments, and visits to friends, family members, and stores. Engage in assistance and planning of client's recreational activities and events. Remind client(s) to take medications. Document and report changes in the client's physical condition, behavior, or appearance to Lead Client Care Coordinator. Adhere to HIPAA regulations.
Qualifications:
Must be at least 18 years of age. Ability to follow and complete directions on task sheet provided and exchange accurate information with clients and staff members. Extensive bending, lifting and standing regularly to perform housekeeping duties. No experience is necessary; nevertheless, employees must complete and pass training which will provide the necessary skills required to complete this job. The training will include but is not limited to understanding the needs of the client's served, such as meal preparation, transportation services, housekeeping, home safety and handling emergencies.
Certifications, Licensure, and Registration:
State required certifications may be required. Possess and maintain a valid Driver's License and maintain valid auto insurance if driving personal vehicle.
