This vital role involves providing compassionate, personalized care to clients in their own homes, promoting independence, safety, and well-being. You will support individuals across various age groups and health conditions, ensuring they receive the highest quality of care in a comfortable and familiar environment. Your energy, empathy, and attention to detail will help create a positive impact every day. Duties Assist clients with daily activities such as bathing, dressing, grooming, and mobility to enhance comfort and independence Provide companionship and emotional support to reduce feelings of loneliness and promote mental well-being Monitor health status, report changes, and coordinate with healthcare professionals as needed Administer medication reminders and assist with light household tasks to maintain a safe living environment Support clients with meal preparation and nutritional needs tailored to individual preferences and dietary requirements Follow care plans meticulously, ensuring all services are delivered safely and effectively Maintain detailed documentation of care provided, client progress, and any incidents or concerns Qualifications Prior experience in home care or related settings is preferred; experience with Level II trauma centers or hospital medicine is a plus Knowledge of emergency medicine procedures and post-acute care practices enhances your ability to respond effectively in urgent situations Strong interpersonal skills with the ability to build trusting relationships with clients from diverse backgrounds Ability to handle physically demanding tasks such as lifting or assisting with mobility safely Certification in CPR/First Aid is highly desirable; additional training in geriatrics or medical-surgical care is advantageous Demonstrated reliability, compassion, and commitment to providing exceptional patient-centered care Experience working within teaching hospitals or outpatient clinics adds valuable insight into varied healthcare environments Join us in delivering compassionate in-home care that empowers individuals to live healthier, happier lives.
