JOB SUMMARY

Provide routine individualized healthcare to the elderly, convalescents, or person with disabilities at the patient's home or in a care facility. Monitor or report changes in health status. May also provide personal care such as bathing, dressing, and grooming of patient.

ESSENTIAL DUTIES AND RESPONSIBILITIES

All duties of the Home Health Aide are under the direction of a Registered Nurse or Therapist as follows: (Other duties may be assigned.) § Assist with personal care, which shall include assisting the client with personal hygiene, bathing, dressing, grooming, ambulation, eating, shaving and physical transfer. § Assist with the change of a colostomy bag and reinforcement of dressing, if instructed in procedure by an RN. § Assist with the use of an aid for daily living, i.e., walker, cane, wheelchair. § Assist with prescribed range of motion exercises which have been taught by a health care professional of the agency. § Take vital signs (TPR) as directed. § Perform catheter care as directed. § Measure intake and output of fluid. § Keep records of personal health care and activities. § Submit patient's notes & paperwork on a weekly basis Supervision of self administered medication in the home or healthcare by: § Remind patient of the time to take the medication as prescribed. § Prepare necessary items such as juice, water or spoons to assist client in the self administration of the medication. § Provide homemaking and light housekeeping of the patient's care area as directed. § Prepare meal services as directed.
