Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and hygiene. Monitor and observe patient health status by checking vital signs, reporting changes, and maintaining detailed medical documentation using Electronic Health Record (EHR) systems. Support medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards. Provide specialized care for clients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies. Assist with mobility and patient transfer methods utilizing equipment such as Hoyer lifts or patient transfer techniques to ensure safety for both patients and caregivers. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for the client. Support end-of-life or hospice care needs with compassion, dignity, and respect for patient preferences. Skills Proven experience working with geriatric patients, individuals with developmental disabilities or disabilities requiring long-term care. Strong knowledge of medical terminology, catheter care, wound management, and basic life support (BLS) including CPR. Skilled in patient observation, medication administration, vital signs measurement, and documenting medical records accurately. Familiarity with EMR/EHR systems for efficient record keeping and communication within healthcare teams. Excellent client communication skills combined with compassionate caregiving abilities to foster trust and rapport. Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or other patient transfer devices. Knowledge of home health care assistance standards, home & community care practices, hospice protocols, and working within home health agency environments.
