Support residents during meal preparation and feeding, ensuring nutritional needs are met while accommodating dietary restrictions. Utilize patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safe movement and positioning. Maintain a clean and safe environment by performing routine cleaning tasks and adhering to infection control protocols. Provide emotional support and engage residents in meaningful activities to promote socialization and mental well-being. Follow individualized care plans developed by healthcare professionals, including dementia care, Alzheimer's care, memory care, and neurodevelopmental support. Document all interactions accurately and communicate effectively with team members regarding resident progress or concerns. Assist with transportation for medical appointments or community outings using appropriate personal transportation assistance methods. Respond promptly to emergencies with first aid or CPR when necessary, ensuring resident safety at all times. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Familiarity with patient observation techniques, medication administration, vital signs monitoring, and care plan implementation. Hands-on experience providing dementia care, Alzheimer's care, or memory support for individuals with neurodevelopmental or intellectual disabilities. Knowledge of HIPAA compliance standards related to patient confidentiality and data security. Proven ability to perform patient transfers using Hoyer lifts or other safe manual handling methods; heavy lifting may be required. Experience working with individuals with autism or developmental disabilities is advantageous. Demonstrated caregiving skills in geriatrics, hospice care, or long-term care settings; home & community care experience is a plus.
