SUMMARY :

Ensure the health, safety and welfare of the adult residents through supervision of activities, assistance with personal goals and fulfilling the group home routine (i.e. meal preparation, daily chores, etc.), as needed.

KEY RESPONSIBILITIES

Support Daily Living:

Provide supervision and assistance to individuals to help them maintain autonomy, perform daily living tasks, and engage in the routines of home life.

Build Meaningful Relationships:

Spend quality one-on-one time with individuals to foster trust and connection.

Goal Achievement:

Support individuals in setting, implementing, and achieving personal goals as outlined in their Individual Support Plans.

Meal Preparation & Home Maintenance:

Assist with preparing nutritious meals and maintaining a clean, safe, and welcoming home environment.

Budgeting & Shopping:

Help individuals manage personal finances and shop for their needs.

Community Engagement:

Encourage and participate in activities and events chosen by the individuals, promoting social inclusion and enjoyment.

Transportation:

Safely transport individuals to medical appointments, day programs, and community events as needed.

Documentation:

Maintain accurate and up-to-date records, including case notes, counseling sessions, health data, and training logs.

Incident Reporting:

Promptly report and document any incidents involving individuals, the facility, or group home vehicles.

Rights & Confidentiality:

Uphold and protect the rights, privacy, and personal information of all individuals served.

Team Collaboration:

Attend staff meetings, Individual Support Plan meetings, and required in-service training sessions as directed.

Additional Duties:

Perform other related tasks as assigned that support the overall mission and function of the role.
