In this vital role, you will provide personalized support to individuals across various stages of health and recovery, ensuring they receive safe, respectful, and effective care. Your proactive approach and empathetic nature will help promote independence, comfort, and well-being for those you serve. This position offers an opportunity to work closely with patients, families, and healthcare professionals in a dynamic and rewarding environment. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report any irregularities promptly Support medication administration following prescribed guidelines and document all medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and behavior management strategies Perform catheter care, wound care, and other medical procedures as trained; ensure proper infection control practices are followed Assist with transfers using patient transfer methods such as Hoyer lifts or gait belts while maintaining safety protocols Support end-of-life care with compassion and dignity in hospice settings; communicate effectively with patients' families about care plans Maintain patient confidentiality by adhering to HIPAA regulations; document all interactions thoroughly in compliance with home healthcare regulations Support patients with light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe environment at home Collaborate with healthcare teams to coordinate care plans and update medical documentation regularly Qualifications Proven experience working as a Home Health Aide or in assisted living/nursing home environments; experience with individuals with developmental disabilities or geriatrics is highly valued Strong knowledge of medical terminology, patient care procedures, and home health care regulations Certification in First Aid and Basic Life Support (BLS), including CPR; additional certifications such as hospice or dementia care are advantageous Familiarity with EMR/EHR systems for documentation purposes Ability to perform heavy lifting when necessary and use patient transfer equipment safely Excellent communication skills to effectively interact with patients, families, and healthcare professionals Compassionate attitude with a genuine desire to improve patient quality of life Valid driver's license and reliable transportation preferred for travel between patient homes Join us in delivering exceptional home healthcare assistance that truly makes a difference!
