This Case is located in Port Saint. He is a 52yr old male with intellectual disabilities. This case is flexible; it is around 17 hours a week and is a companion case. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts when necessary. Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring timely reporting of any abnormalities. Administer medications as prescribed, following medical documentation protocols and adhering to medication administration guidelines. Provide companionship and emotional support to promote mental well-being while respecting patient preferences and cultural sensitivities. Requirements This is an APD case; it is required to have Train certifications. Proven experience as a Home Health Aide or in similar caregiving roles within home healthcare agencies or long-term care facilities. Demonstrated proficiency in patient observation, medical terminology, vital signs monitoring, medication administration, and basic life support (BLS), including CPR certification. Experience working with individuals with developmental disabilities, dementia or Alzheimer's disease, geriatrics, or hospice care is highly desirable. Knowledge of home & community care practices along with familiarity with home health regulations and documentation standards. Ability to perform heavy lifting when necessary and operate patient transfer equipment such as Hoyer lifts safely. Strong interpersonal skills to communicate effectively with clients and their families while maintaining patient confidentiality per HIPAA guidelines. Certification in first aid is required; additional certifications in dementia or Alzheimer's care are advantageous. Flexibility to work live-in employment arrangements or varied shifts based on client needs; reliable transportation is preferred.
