Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, recording vital signs such as blood pressure, pulse, and temperature accurately Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA and patient confidentiality standards Utilize Electronic Health Record (EHR) systems to document patient information, care plans, and medical records efficiently Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management Assist with catheter care, wound dressing, and other medical procedures as directed by healthcare professionals Support patients during transfers using appropriate patient transfer methods and equipment like Hoyer lifts or gait belts Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation Follow home health care assistance protocols for hospice or end-of-life care with compassion and professionalism Communicate effectively with clients' families and healthcare teams to ensure coordinated care Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred Demonstrated experience working with individuals with developmental disabilities or geriatrics is advantageous Knowledge of medical terminology, medical documentation practices, and home health regulations is essential Hands-on experience with EMR/EHR systems for accurate record keeping is desirable Certification in first aid, CPR (Cardiopulmonary Resuscitation), and basic life support (BLS) is required or obtainable upon hire Experience working with clients requiring hospice or dementia care enhances your candidacy Ability to perform heavy lifting safely and effectively when assisting patients or handling supplies Join our team to make a positive impact through compassionate caregiving!
