PRINCIPAL RESPONSIBILITIES

Personal Care and Daily Living Assistance

• Provide individualized care in accordance with client care plans, including assistance with grooming, bathing, oral hygiene, toileting, feeding, dressing, mobility, and use of prosthetic devices.
• Support clients with communication needs and perform range of motion exercises as directed.
• Maintain skin integrity through proper hygiene, repositioning, and monitoring for signs of pressure injuries or skin breakdown. Observation, Documentation, and Reporting
• Observe and accurately document changes in clients' physical condition, behavior, or mood.
• Promptly report significant findings, incidents, or suspected abuse, neglect, or rights violations to the charge nurse or designated personnel, following established policy.
• Record attendance and verify hourly client counts using the BAFI Live Kiosk to ensure accountability. Safety, Security, and Infection Control
• Follow all infection control procedures, including proper hand hygiene, use of gloves, and disposal of soiled materials.
• Safeguard clients' personal belongings and report any missing items per established procedures.
• Maintain a clean, safe, and organized environment by performing light housekeeping and reporting maintenance needs.
• Adhere to all safety protocols to prevent accidents and ensure a secure care setting. Mobility and Transportation Support
• Safely lift, move, and transfer clients using approved body mechanics or assistive devices.
• Assist clients with mobility within the facility and when boarding or exiting BAFI vans, ensuring comfort and safety at all times. Clinical Support
• Perform delegated clinical tasks under supervision, such as obtaining vital signs, weights, and specimens; applying topical treatments; and emptying urinary catheters. Teamwork and Professional Conduct
• Collaborate effectively with coworkers and supervisors, demonstrating reliability, professionalism, and sound judgment.
• Communicate respectfully and tactfully with clients, families, and staff, particularly in sensitive or emotional situations.
• Promote a positive, welcoming environment for clients and visitors, representing BAFI with empathy and integrity. Physical Requirements
• Must be able to lift, move, or carry up to 50 pounds as part of daily responsibilities.

Additional Responsibilities Performs other related duties as needed to support the overall operations and success of the organization.

QUALIFICATIONS

Required

• Possession of a valid Home Health Certificate
• High school diploma or equivalent (GED)
• Current and valid CPR certification
• Medical clearance from a licensed physician verifying the absence of communicable diseases, a negative TB test, and the physical ability to perform essential job duties
• Ability to pass a Level 2 background screening
• Ability to pass required drug testing Preferred Previous experience working with the elderly and/or a medical aide background
