Job Description

Job Summary Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or wheelchairs Monitor and record vital signs like blood pressure, pulse, temperature, and respiration to track health status accurately Support medication administration following prescribed guidelines while maintaining compliance with HIPAA regulations for patient privacy Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques and behavioral management Perform patient observation to identify changes in condition or behavior and communicate findings promptly to healthcare teams Assist with medical procedures such as catheter care, wound dressing changes, or hospice support as directed by medical professionals Maintain detailed medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure accurate record-keeping and compliance Support household tasks including light cleaning, laundry, meal preparation, and ensuring a safe environment for patients in home and community settings Work with individuals with developmental disabilities or disabilities by providing tailored care plans that promote independence and quality of life Follow all safety protocols during patient care activities and participate in ongoing training such as CPR certification or medical terminology updates Qualifications Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments preferred Strong knowledge of patient observation techniques, vital signs measurement, medication administration, and medical terminology Experience working with individuals with disabilities or developmental challenges is highly desirable Certification in CPR and first aid is required; additional certifications such as hospice care or catheter care are a plus Familiarity with EMR/EHR systems for documentation purposes; ability to accurately maintain medical records and logs Compassionate caregiver with excellent communication skills who thrives in a team-oriented environment Ability to work independently while managing multiple patient needs in a home setting Flexibility to work live-in shifts or flexible hours as needed to support patient needs Join us in delivering exceptional home health services that empower individuals to live healthier, happier lives. $18.00 - $20.00 per hour
License/Certification:
